The goal of UCP Chicago's
adult services is to develop options for adults with severe disabilities
so they may exercise as fully as possible their right to participate
in the community of their choice.
UCP's adult programs stimulate participation and skill development
through structured learning and provide support through individual
and group counseling. Always, the aim is to determine and fully
realize the physical, social and intellectual potential of each
participant.
Each individual, age 18 or older, is accepted into the program
based on an initial evaluation of his/her needs and personal goals
and desires, from which an Individual Service Plan (ISP) is developed.
The ISP outlines specific, attainable goals for the participant,
around which his/her daily activities are scheduled. Goals, action
steps, and progress are evaluated every six months and modified
as needed.
Round-trip bus transportation is provided between each participant's
home and the program site.
